We know that you have many choices where to refer your client for treatment and we appreciate your confidence in choosing New Bridge Foundation. We don’t take that for granted and we will work very hard to maintain that trust you have in us. When referring someone to New Bridge Foundation, here is what you can expect:
Seamless process.
Our goal is to insure an effective and efficient transition for your client to enter into our program and then back again into the community. This process includes:
Accessibility: The initial assessment can be scheduled within 24 hrs of your call. Admission can often occur the same day.
Free Assessment: The initial assessment to determine medical necessity and appropriate level of care is free of charge.
Responsive Communication: We will obtain a release of information from the client and be able to communicate with you from the initial telephone call through aftercare. Whether you want a weekly phone call or just a summary at discharge, keeping you informed of your client’s treatment is important to us.
A Dedicated Case Manager: We will provide a Dedicated case Manager for your client and a Utilization Review staff member is available to update you on your client’s progress.
